The role of medical center needs innovating with the
development of the times. The medical center can't only focus
on critical care, but also on Long-term care. Long-term care for
people with chronic illnesses and disabilities presents an urgent
challenge in Taiwan. In ideal condition ,when patient
admissions in hospital ,we should start comprehensive
evaluation from the patient to the family to identify patient’s
and family’s continuity care needs, and provide the right
services according to the needs of patients. Most medical
centers in Taiwan have been actively involved Long-term care,
but the effectiveness of the service is limited by the lack of
professional manpower. So how to connect with society, and
interprofessional collaboration and teamwork capability of
long-term care to and development of health care professionals
is important.

In the other hand, the middle-aged providing for the
elderly is a care model that’s gradually becoming the norm in

Taiwanese society, so caregiver health is a crucial public health
concern due to the increasing number of elderly people with
disabilities. Capacity is a key determinant of the ability to
provide care and mechanisms are needed to assess the strength
and limitations of family caregivers and to intervene with
support and respite as needed .How best to provide social
