In order to arrange an appointment with Chang Gung Craniofacial Center, please complete the Appointment Request Form.
The Center will respond to your request by email or fax within a few days.
If possible, please be ready to provide additional medical information for our specialists. This information will help our physicians to better understand your health condition and to better prepare for any personalized details of your procedure.
Documents that are likely to be helpful include, medical records, imaging studies （X-ray, MRI, CT scans, ultrasound）, and pathology slides. These can be sent to the Center via expedited mail or secure e-mail.

Once our specialists review and accept your case, a patient coordinator will confirm your appointment date, transportation details from CKS airport to the Craniofacial Center, hotel reservations, and any necessary visa requirements. The patient coordinator assigned to your case will also assist with all arrangements related to your travel to the Center.
