Functional Grading of balance

Balance grade	Definition
Zero(Unable)	Patient needs maximal assistance to maintain balance
Poor	Patient needs support to maintain balance （所以functional status為poor，一定還要註明需要多少程度的support）
Fair	Patient is able to maintain balance without support; cannot tolerate challenge; cannot maintain balance while shifting weight (weight shifting test: OT是請病人拿取60cm前方地上的沙包)
Good	Patient is able to maintain balance without support; accepts maximal challenge and can shift weight, although limitations are evident
Normal	Patient is able to maintain balance without support; accepts maximal challenge and can shift weight in all directions

Functional independent level

Supervision	Subject 100% （病人自己出100%的力）
Minimal assistance	Subject 75~100% （病人自己出75~100%的力，也就是扶助他的人需出25%以下的力）
Moderate assistance	Subject 50~75%
Maximal assistance	Subject 25~50%
Total assistance or not testable	Subject 0~25%
