All applicants must provide a health screening report issued within 3 to 6 months, including:
If antibodies are not present, you must submit proof of vaccination or a certificate of contraindication issued by a regional hospital or higher, recognized by the Ministry of Labor.
You may choose to complete the screening at Chang Gung Memorial Hospital (self-paid) at least 10 days before your training start date.
During periods of infectious disease outbreaks, additional vaccination records or screening reports may be required based on the hospital or national policy.
