Upon arrival, please report to your department secretary to collect your trainee ID badge and access card.
A NT$500 deposit is required. The deposit will be refunded upon return of the items at the end of your training.
If you have applied for accommodation, the secretary will guide you to the dormitory. For dorm-related questions,
contact the dorm supervisor (Ext. 3900, Mobile: 0975-368-349). The room key must be returned before moving out.
After completing administrative procedures, please contact your supervising physician to confirm your training schedule and objectives.
Training Evaluations:
Training ≤ 3 months: Submit a learning assessment before completion.
Training > 3 months: Assessments must be conducted every 3 months.
Certificate of Completion: Issued only if all evaluations are completed and the trainee performs satisfactorily during the program.

Q: Can I get the deposit back if I lose my ID badge? A: No. The deposit is only refunded if both the trainee badge and access card are returned.
Q: Who should I contact if my dorm key is lost? A: Please notify the dorm supervisor immediately (Ext. 3900 / Mobile: 0975-368-349). You will also be required to pay for key replacement; please confirm details with the dorm supervisor.
Q: Can I apply for a certificate without submitting the assessment? A: No. All required evaluations must be completed to receive a certificate.
Q: Can I extend my training period after arrival? A: Please discuss with your supervising physician and notify your department secretary at least 2 months in advance. This allows sufficient time for internal processing and reporting to the Ministry of Health and Welfare.
